Q: I have a family member that needs extra supports at home in order to be able to stay there longer, can you help?

A: YES! Our Care Team can coordinate with you so that we can offer an in-home intake meeting where we work together to determine what extra supports are required and then we can help to take the burden of care off your family, so that your loved one is safe and secure at home for a longer period of time.

Q: My husband and I are getting older, and we have concerns about the ability of our Son (who has Down Syndrome) to be able to live more independently on his own when we are unable to care for him any longer. Can you help?

A: Our Care Coordinator Team can meet with your husband, your son and yourself to support and determine which extra care supports that he may benefit from. We may be able to help strengthen his community programs, support him with becoming more independent with transit / home and life skills and basic meal preparation. There may be many areas that our team can support your family with. Let's work together to help reduce some of your stress.

Q: My Mother is currently in an Independent Living senior's facility, but she is starting to need more help with things. I live out of the city, and can't be there to support her with her growing needs. How can you help?

A: We can work together to assess your mother's needs and change with her as those needs may change. Even in independent living facilities, many residents require additional supports in order to continue residing there. That's not a problem. Let us provide a consultation with her to determine what needs she may require, and we may even be able to liaise with the residence to get some extra supports put in place there too. Whether she needs extra PSW care, or reminder programs set up for medications, or companion care to keep her from feeling depressed, Evolution's Care Team can definitely help.

Q: I am the General Manager at a Retirement Residence, and our staffing has become an extra task that I struggle to keep up with at times. If I need help with bringing in extra support staff for my dining area or my PSW workers, can you help?

Q: My daughter has anxiety and Asperger's. She is becoming more and more reclusive and doesn't want to leave the house.  I am exhausted and feel overwhelmed and I am not sure how to offer some extra strength to try to support her with getting back out in to the community. Can Evolution help?

A: Our Care team has experience working with many clients that struggle with Anxiety and understand how difficult these can be, not only for our clients, but for their families. We can work together to match your daughter with a Care Team member that can work with her to support a growth in confidence and hopefully a reduction in some anxiety. Stretching boundaries and providing not only some respite care for your family, but some growth and experience strengthening for your daughter.

Q: How do I know that I can trust you and your workers?

A: Evolution Care Team members are screened and reference checked. All team members must provide a current Police Check with vulnerable sector screening. Q: I am a PSW that currently works part-time in a care facility, but I am seeking opportunities to expand and grow my work experience with limited availability due to my other job commitment.  Is there a spot for me to join your team?

Q: My Dad is in hospital and getting released shortly, but CCAC has advised that they can only provide a small amount of support hours for him once he gets back home. I am worried that he will need more help in order to recover. Are you able to combine your pay-per-service supports with what we are provided through CCAC?

A: Many families face the same challenges -- and yes, what you should do is confirm with your CCAC Case worker exactly the supports that you will received through them. Then we can have a consult to determine what additional supports you feel that your dad may need so that we can help to fill the gaps. If your dad's needs change -- then we can support that too. Coming out of hospital can be a very stressful time for seniors and their families, so we are here to help.
